Mindmyhealth.org is a project of Health Sciences Health Innovations Group (HSHIG). HSHIG understands your privacy is important and is committed to protecting the privacy and security of your personal data. This Privacy Policy explains how we handle personal data collected when you use our website www.mindmyhealth.org on any platform and in any format (this “Website”).

Data We Collect and How We Use It

We collect personal data that you provide through voluntary submissions via this Website, for example, by registering an account, submitting your advance health care directive or other advance planning document and using our service. Our service prompts you to upload and submit to us and to a participating hospital system your advance health care directive or other advance care planning document, which necessarily involves the submission of personal information, personal preferences and sensitive end-of-life topics. If you do not want us to collect this information, do not use our service. This Website also collects personal data from you when you request information from us and submit an inquiry or comment to us online.

Data we collect may include:

  • Your name, email address, state of residence, zip code, phone number, birthday, gender, device IP address and location.

  • Your hospital system selection, organ donor status, advance health care directive or other advance care planning document and the date of document activation.

  • Any information you submit to us via our contact form.

  • The name and email address of a Care Contact, who is a person you may authorize to access your Mind My Health account on your behalf.

We use this data to provide our services; to deliver and make available your advance health care directive or other advance planning document to hospitals, hospital systems and health care providers; to you respond to your inquiries and requests; to deliver newsletters, educational marketing and other information to you; for research purposes; and to analyze Website usage and improve our Website and to personalize content.

We also collect data through the use cookies and analytics as described below.

 

How We Share Data

HSHIG will not share your personal information except in the following limited circumstances:

  • We will share your personal data and your advance health care directive or other advance care planning document with our third-party verification service provider Cloud Factory. Cloud Factory has its own privacy policy, which we encourage you to review.

  • We may collect, report and share personal data with other third parties that perform services on our behalf, for example, our technical, promotional and content service providers. We may also share your personal data with third-party providers of cloud, web and/or server hosting services. We do not authorize these third parties to use your personal data except as necessary to provide services to us or for purposes authorized by us.

  • Our Website may also use third-party service providers for the following purposes: (1) to create and manage our email lists to fulfill newsletters, educational marketing and other purposes, (2) to handle your comments and questions submitted on our Website, (3) to deliver educational marketing and related material, (4) to analyze navigation and site usage patterns and (5) to store and process data.

  • We may share your personal data and your advance health care directive or other advance care planning document with grant administrators who are evaluating our service.

  • Additionally, we may disclose your personal data (1) to comply with a law, regulation or other governmental request, for example, to comply with subpoenas, court orders, legal process or requests by law enforcement or governmental entities; (2) to investigate or defend against third-party claims against us; (3) to detect, prevent or investigate fraud or other illegal activity or to enforce our Terms (www.mindmyhealth.org/legal); (4) to repair or resolve technical issues in the operation of this Website; (5) to respond to a request by you to disclose the data, including your requests to allow access to your account and personal data by a Care Contact or other third-parties designated by you; (6) with your knowledge and consent; and (7) to address emergencies or acts of god. We may share your personal data as an asset of the organization in the event of a sale, merger or other entity combination or transfer involving us, our parent or subsidiary entities.

Our third-party partners and service providers may collect personal data from you as authorized by us. Our service providers may provide data collected by them through this Website to us, such as reports of email addresses and personal data they collect, store and/or process for us, which we may use for educational marketing purposes. We are not responsible for the privacy practices of these service providers or of other websites.
